Chapter headings
This table of contents has headings for each of the chapters. Each heading can be treated as a new section, with two blank lines before it. The four/two spacing around "CONTENTS" works both for designating the major division and the minor division. Despite the use of hanging indents, no-wrap tags are still used in conjunction with the six spaces before page numbers in order to indicate alignment.
