Per-cell markup

Each cell of a table is formatted separately, so the "hors de combat" in italics should be marked as shown: a single phrase, with one pair of tags; and not as two italicized phrases, even though it's on two lines. This applies even when the text is on more than one line of a "cell" in the original image. The text also should be aligned based on how it will look in plain text (see below).

Below is how the markup above will appear in plain text: the columns are aligned and "hors de combat" is properly italicized across two lines in its cell.
