THE GEOGRAPHY OF ITALY

Italy, ribbed by the Apennines, girdled by the Alps and the sea,
juts out like a "long pier-head" from Europe towards the northern
coast of Africa. It includes two regions of widely differing physical
characteristics: the northern, continental; the southern, peninsular.

Book titles

A book title is a major division: four blank lines go before it. Note that here, "A.D." is shown in normal uppercase, not in small caps.

Chapter spacing

Use another four blank lines to mark the actual chapter, one line between the parts of the chapter heading, and, finally, two blank lines to mark the start of the text.

Inline markup in chapter titles

The formatting guidelines say:

While chapter headings may appear to be bold or spaced out, these are usually the result of font or font size changes and should not be marked.

Here, the title ("THE GEOGRAPHY OF ITALY") is not bold, not small caps, just all uppercase.
