Book titles
Book Title is Major Division: 4 blank lines. Note that here, "A.D." is shown in normal upper-case, not in small-caps.
Chapter spacing
Use another 4 blank lines to mark actual Chapter, one line between the parts of the chapter heading, and, finally, 2 blank lines to mark start of text. (No discussion yet for this example)
Inline markup in chapter titles
The Formatting Guidelines say:
While chapter headings may appear to be bold or spaced out, these are usually the result of font or font size changes and should not be marked.
Here, the title ("THE GEOGRAPHY OF ITALY") is not bold, not small-caps, just all upper-case.
