When formatting an index, regardless of how it is formatted in the original text, we:
- Surround the index in /*no-wraps*/ (even if it looks wrappable)
- Rejoin any entries that have been split across two lines
- Separate the entries and the list of page numbers with a comma if there isn't one already
- Place one blank line before each main entry
- Start each sub-entry on its own line, starting with two spaces
