An index.

When formatting an index, regardless of how it is formatted in the original text, we:

  1. Surround the index in /*no-wraps*/ (even if it looks wrappable)
  2. Rejoin any entries that have been split across two lines
  3. Separate the entries and the list of page numbers with a comma if there isn't one already
  4. Place one blank line before each main entry
  5. Start each sub-entry on its own line, starting with two spaces
